Ultimate Exposures Consent Disclaimer (for registering players). 

I hereby authorize AYSO Region 121 to release the following information to Ultimate Exposures: The athlete’s first name, last name, team name, division, and parent’s email address and mailing address. This data will only be utilized to email proofs to the families for the sole purpose of purchasing pictures, image downloads, specialty products and mailing picture packages. Ultimate Exposures will not divulge any athlete, coach, or parent data to outside vendors or any additional sources. This data includes names, street addresses, or email addresses given to them either by the league or the customer directly.  By signing, I understand that my information will be sent via mail, e-mail or secure portal.
